Plasma HIV RNA is monitored frequently during pregnancy to confirm attainment and maintenance of
viral suppression. Close monitoring is desired to confirm the adherence to and effectiveness of the
patient’s ART regimen since higher levels of HIV RNA are associated with increased risk of perinatal

Prior to delivery, the HIV­infected woman should be counseled that breastfeeding is not recommended by
the United States Public Health Service, due to concerns regarding risk of breast milk transmission of
HIV. (See 'Counseling regarding breastfeeding' above.)
